For temporary service disconnection due to service repair or upgrade, please complete the form below.

An approved meter enclosure will be determined by KLPD.  No ring-style meter enclosure will be allowed.

After completion, a representative from KLPD will contact you to confirm the requested date of service. Although every effort will be made to accommodate your needs, it may be necessary to reschedule due to previously scheduled work, inclement weather, or situations emergent in nature.
